Alberta-Wide 24/7 Mental Health Helpline
The Alberta Health Services Mental Health Help Line is a 24 hour, 7 day a week, 365 days a year, confidential service that provides support, information and referrals to Albertans experiencing mental health concerns. The line is staffed by a multidisciplinary team comprised of nurses, psychiatric nurses, social workers, occupational therapists, and psychologists. The service provides crisis support, mental health or addiction screening and assessment over the phone, general information about a mental health or addiction topic, information about local service options for addiction or mental health and how to access them, and strategies people can use at home to support their emotional well-being. Well on Your Way: Helping Youth Transition to Adult Healthcare
This service helps youth and their families prepare for the transition from pediatric to adult healthcare. This service offers the following transition to adult healthcare supports for patients and their families: -Well on Your Way website – Provides information, tools and resources for youth, families and healthcare providers regarding the transition to adult healthcare -Teens Talk Transition – Workshops to help youth and their families prepare for the transition from pediatric to adult healthcare. These workshops run for three consecutive Saturdays and are offered twice per year. For information about upcoming workshops inquire by email.

Adult Day Program for Individuals with Physical/Memory Challenges or Chronic Illness - Home Care
Adult day programs are designed for people over the age of 18 who may have physical and/or memory challenges, or are living with a chronic illness. The programs play a key role in allowing people to remain living in the community as long as possible by optimizing their level of physical, spiritual, social and emotional functioning. **Those who live in supportive living, long‑term care or are receiving funding from Persons with Developmental Disabilities are not eligible.**

Community Care Access Program - Calgary
Central Intake for referral to Calgary Zone Home Care, Supportive Living and Long Term Care. This service provides a single point for service access and delivery approach that provides Albertans with reasonable, timely and appropriate access to publicly-funded continuing care health services. It is based on availability and determination of unmet need. Anyone living in Alberta with a valid provincial health care card (or registration in progress) can receive Home Care services as long as their health care needs can be safely met in the home.
